You get 3 tries per permit.
Form DL-180C, if you're under 18
Only if it applies
A parent certifies your 65 practice hours: 10 at night and 5 in bad weather.
The car's registration card and insurance
Both current, for the car you'll drive.
The adult driving with you, and their license
A licensed driver 21 or older, or a licensed parent, guardian or spouse 18 or older.
A car in good order
Current inspection and emissions stickers, and working lights, brakes, horn, wipers, signals, mirrors, doors, seats and tires.
