Proof of your Social Security number
Your Social Security card or another accepted document.
Proof of citizenship or legal presence
A US passport or birth certificate covers this.
2 proofs of your Ohio address, from different sources
Utility bills, bank or credit card statements and similar, showing your Ohio street address.
Name change papers, if your name changed
Only if it applies
A certified marriage certificate, divorce decree or court order linking your names. Several marriages may need one for each.
