One proof of age and identity
A certified birth certificate, a valid U.S. passport or a green card, for example.
One proof of your Social Security number
Your card, a W-2 or a pay stub showing it.
Two proofs of Maryland residency
Two documents with your name and Maryland home address, such as a utility bill, lease, insurance policy, credit card statement or voter registration card. Originals only.
Name change papers, if your name differs
Only if it applies
A marriage certificate, divorce decree or court order.
