Proof of identity
A birth certificate, U.S. passport, green card, Consular Report of Birth Abroad, or certificate of naturalization or citizenship.
Proof of your Social Security number
Your Social Security card, a pay stub or a W-2 or 1099 showing it.
2 proofs of Maryland residency
Two documents with your name and Maryland home address, such as a utility bill, lease, insurance policy, credit card statement or voter registration card. Originals only.
Under 16: form DL-300 from your school
Only if it applies
In a sealed envelope; DL-300A for home school.
Under 18: a parent or guardian to co-sign
Only if it applies
With proof of the relationship.
Payment, $65
Covers converting the permit to a license later.
