Proof of citizenship or lawful presence
A U.S. birth certificate, valid U.S. passport, green card or certificate of citizenship or naturalization, for example.
Proof of your Social Security number
Your Social Security card, a W-2, 1099 or pay stub showing the full number.
2 proofs of Tennessee residency
Two documents with your name and Tennessee address, dated within the last 4 months: a utility bill, bank statement, mortgage, or a lease with a recent landlord letter, for example. No photocopies. Teens can use a parent's.
Under 18: your school attendance form
Only if it applies
Form SF1010 from your school, showing you're enrolled and attending.
Under 18: a parent or guardian, with proof of insurance
Only if it applies
To sign the Minor/Teenage Affidavit in front of the examiner, or a notarized copy.
Payment
$10.50 under 18, $5.50 from 18. $2 each time you fail a test.
