1 proof of identity
A US passport, US birth certificate, Permanent Resident Card or another listed document, showing your full name and date of birth.
Your Social Security number
Needed for most applicants; California DMV lists a few exceptions.
2 proofs of California residency
2 different printed documents with your name and the same mailing address, like a utility bill, bank statement, lease or insurance document.
Name change papers, if your name changed
Only if it applies
One document for each change: marriage certificate, adoption papers, court order, or dissolution of marriage or domestic partnership.
