One proof of identity
An original or certified U.S. birth certificate, or an unexpired U.S. passport or passport card. Other lawful presence documents are on MVD's list (form 40-5144).
Your Social Security number
No card needed, but you need the full number.
Two proofs of your Arizona address
Mail from a business, organization or government agency with your name and home address, such as a utility bill, bank or credit card statement, insurance policy or rental agreement, from two different sources. If you have only one, an Arizona Residency Affidavit (form 40-5143) can count as the other.
Proof of any name change
Only if it applies
A marriage certificate, divorce decree or court order linking your names.
